Pursuant to the Notice of Annual General Meeting issued 19 July 2023 this Form is to be used to Register to attend (In Person or via Zoom) and receive the Agenda Papers for the Annual General Meeting or Register as a Proxy Vote.

When you Register select if you will be attending in person or remotely via Zoom or will be unable to attend either way and require a Proxy Vote.
Using the information you provide when registering the Club will verify you are entitled to attend and vote.
The Annual General Meeting Agenda, Zoom Details (Where Required) and Proxy Vote Form (Where Required) will then be emailed to the email provided in the Registration Form below.

Registration for a Proxy Vote must be received no later than twenty-four (24) hours before the commencement of the meeting.
